Summary

Our project focuses on improving implementation of bedside handoff. Research shows that bedside handoff significantly improves patient satisfaction, safety, and quality of care. However, despite its benefits, implementation remains low due to concerns about confidentiality, lack of confidence, and time constraints. To address these barriers, we propose a four-month phased implementation starting May 1st. This includes introducing a new bedside handoff tool, developing an education module, ensuring completion of training, conducting in-person mock trials, and launching audits by September 1st. Our goal is a 50% increase in bedside reporting, leading to improved patient satisfaction, lower care costs, fewer sentinel events, and reduced nursing errors from miscommunication.

Additionally, we will implement a tiered handoff reporting system to enhance communication and efficiency. This structure prioritizes information into three levels: Tier One covers essential details like patient identity, code status, allergies, and care plans; Tier Two includes medical history, medications, mobility status, and diet; Tier Three addresses sensitive concerns such as patient aggression or family dynamics. Organizing handoff data in this manner improves interdisciplinary communication, ensures critical information is relayed accurately, and empowers patients by involving them in their care. This structured approach simplifies bedside reporting, promotes compliance, and ultimately enhances patient outcomes.
