
OTD Capstone Projects
Abstract
The Center for Living & Learning is a residential and vocational mental health facility for individuals with persistent mental illness who cannot live independently. They have three levels of care on the property: 24-hour direct care, semi-independent living & independent living. This doctoral capstone project aims to partner with The Center for Living & Learning and develop transitional programming to support the cultivation of independence in residents who have the potential to transition to a less supported environment. Outcomes include a comprehensive 42-page care plan that outlines expectations for direct care, expectations for semi-independent living, how to bridge the gap as well as site specific assessment tools, interventions, handouts, activities, various resources and deliverables.
